Prescription required. Treatment is available only if a licensed clinician determines it is medically appropriate. No prescription is guaranteed. Not everyone qualifies.

Our Clinicians

Every prescribing decision at Reborn Rx is made by a clinician who holds an active, unrestricted license in the state where you are located — and whose name, credentials, and license number you can verify yourself.

In plain language

Care is delivered by [MEDICAL GROUP NAME], a physician-owned practice. Reborn RX LLC provides the technology and support, and does not practice medicine or influence clinical decisions.

No clinician is paid more for prescribing, or for prescribing a particular product. Their compensation does not change based on what they decide.

You can see your clinician's name, credentials, and state license number in your portal, and look it up on your state board's public register.

01Who provides your care

Medical services are provided by [MEDICAL GROUP NAME] and affiliated independent physician-owned professional entities (the "Medical Group"). The Medical Group is owned and controlled by licensed physicians, consistent with the corporate practice of medicine doctrine in the states where it operates.

Reborn RX LLC provides technology, scheduling, billing, and administrative support under a management services agreement. It does not employ prescribing clinicians, does not own the Medical Group, and has no authority over clinical decisions.

Why this matters to you. The separation is not a formality. It means the entity that benefits commercially from a sale is structurally prevented from deciding whether you get a prescription. Your clinician answers to their licensing board and to the Medical Group's clinical leadership — not to Reborn Rx.

02Licensure and credentialing

Before any clinician sees a Reborn Rx patient, we verify:

  • Active, unrestricted state license in every state where they treat patients, confirmed by primary source verification with the issuing board;
  • Education, postgraduate training, and board certification, verified at the source;
  • Current DEA registration where applicable;
  • A query of the National Practitioner Data Bank;
  • Screening against the OIG List of Excluded Individuals and the SAM exclusion list;
  • Malpractice history and current professional liability coverage;
  • Work history and professional references; and
  • A criminal background check.

Credentials are re-verified every two years, and license status is monitored continuously through automated board checks between cycles. A clinician whose license lapses, becomes restricted, or is subject to disciplinary action is removed from patient care immediately pending review.

You are matched only with a clinician licensed in the state where you are physically located at the time of your visit — not where you happen to live or where your card is billed.

03Clinical independence

The following commitments govern how care is delivered:

  • No clinician compensation is tied to prescribing. Clinicians are paid for their clinical time. Their pay does not increase if they prescribe, does not vary by which medication they choose, and is not linked to revenue, conversion, or retention.
  • No quotas or targets. There is no approval rate a clinician is expected to hit, and declining to prescribe carries no consequence.
  • No commercial override. No Reborn Rx employee can review, reverse, or appeal a clinical decision.
  • No manufacturer influence. The Medical Group accepts no payments, samples, sponsorship, or speaking fees from pharmaceutical manufacturers, and no manufacturer has input into treatment protocols.
  • No manufacturer money, and no per-prescription pay. Reborn Rx receives no rebate or payment from any manufacturer, and no clinician is paid more for issuing a prescription. You may direct your prescription to a pharmacy of your choosing.

04Who is on your care team

Prescribing clinicians
Physicians (MD, DO), nurse practitioners (NP), and physician associates (PA), practicing within their state scope of practice and with physician collaboration or supervision where the state requires it. Most hold board certification in family medicine, internal medicine, or endocrinology.
Registered dietitians
Credentialed RDs who provide the nutrition and lifestyle guidance included in the program.
Clinical pharmacists
Review complex medication regimens, drug interactions, and titration plans.
Patient care coordinators
Handle scheduling, shipping, insurance, and billing. Coordinators are not clinicians and never give medical advice — clinical questions are always routed to your clinician.

You may be treated by more than one clinician over time; your complete record is available to whoever is caring for you. You may request a different clinician at any time, without giving a reason — message your care team or call us.

05How to verify your clinician

The name, credentials, and state license number of every clinician who treats you appear in your patient portal on each visit note, and on any prescription you receive.

To check a license yourself

  1. Find the clinician's full name and license number in your portal.
  2. Go to the public license verification service run by the medical, nursing, or physician associate board in your state.
  3. Search by name or license number. The register will show license status, issue and expiry dates, and any public disciplinary action.

The Federation of State Medical Boards also operates a national physician lookup at docinfo.org. If you cannot find the right board, call us and we will send you the direct link — we will never discourage you from checking.

06Quality and oversight

  • Peer review. A sample of charts from every clinician is reviewed each quarter by clinical leadership against written protocols.
  • Protocol governance. Treatment protocols are written and approved by the Medical Group's clinical leadership, reviewed at least annually against current evidence and labeling, and are not editable by Reborn Rx.
  • Safety monitoring. Adverse events are tracked, trended, and reviewed at a monthly safety meeting. See Report an Adverse Event.
  • Continuing education. Clinicians maintain state CME requirements and complete internal training on cellular metabolism, compounded injectable prescribing, evidence appraisal, and telehealth practice standards.
  • Complaints. Any concern about a clinician can be raised with clinical leadership at info@rebornrx.health. You also have the right to complain directly to your state board, and we will provide the details on request.
